• 2018-2019 Freeburg Community High School Registration

New registration procedures for enrolling at Freeburg Community High School
starting with 2018-2019 school year are being implemented this summer. Parents and
guardians should read through all of the information below to identify which actions they
need to take to be prepared for the start of school. All registrations procedures must
be completed prior to students being able to attend on the first day of school.

Office Registration: The office will officially be open for registration July 30-August 2 from
7:45am-11:30am and 1:00pm-3:30pm. There will also be evening hours on Tuesday, July 31
and Thursday, August 2nd from 5:00pm-8:00pm. At this time, returning students and incoming
freshmen should plan on completing their registration process and picking up their schedules by
turning in all additional required paperwork that cannot be collected online. Please review the
sections below for information regarding the required forms.

    Returning 10-12 Grade Students
Residency Change: If you have moved to a new residence in the district since last year, you
will need to prove residency again. The list of items for residency are listed under the Incoming
    9th grade section.

Athletics: All students participating in IHSA athletics are required to have a new physical
each year. In order to try out for any athletic teams, the student must have a current IHSA
physical on file. A copy of the IHSA physical form can be located on the school website. The
physical must be completed on the IHSA form. The steroid and concussion sign off will be
completed during the online portion of registration. IHSA physicals for returning students may be
    turned in at registration.

Vaccinations: All required vaccinations must be completed and documentation must be
presented to the school prior to the student attending school this year. Students will receive
an unexcused absence for each day they are not in attendance due to missing vaccinations.
This is a change in policy from last year. Please check with your doctor’s office as soon as
possible to make sure your shot record is up to date. If you are receiving a vaccination this
year, you must present documentation of that vaccination to the office during registration.
Senior students have been notified by our school nurse regarding their meningitis vaccines
    being due.

Guardianship: If applicable, proof of guardianship. (Divorce decree, court orders,
    agreements or judgement)

Additional Forms If Needed: These forms can be located on the school website and should
be turned in during registration. (Parking permits, Free and Reduced lunch applications,
    Affidavit of Residency.)
